Instructions for Filling the Form

• Username: Enter Employee Number prefixed with "E". Example: E123456
• In case of spouse and dependent Username will start with "N01E" + Staff No of ex-employee
• Date of Separation: Enter the date of Separation in DDMMYYYY format without spaces or slashes. Example: 31052020
• Before filling the form keep the demand note payment slip with you. Payment to be done on bslcollect -> Estate - Demand Note Payment
• This scheme is only for E-type quarter under his occupation (except damage block declared by TA-Civil)
• Keep the Demand Note payment Slip before applying. The form data will not be reset once data is submitted.
